Antecedentes y objetivo Las propiedades biomecánicas corneales en ojos de pacientes recién diagnosticados de glaucoma e hipertensión oculares (HTO) y que no han recibido tratamiento previo son de gran interés, porque pueden suponer una fuente de error en la medida de la presión intraocular (PIO) medida mediante tonometría de aplanación Goldmann (TAG) y Ocular Response Analyzer® (ORA). El principal objetivo de este estudio es evaluar los valores de la PIO obtenidos con la TAG y el ORA, en pacientes con glaucoma primario de ángulo abierto (GPAA) e HTO que no han recibido tratamiento antiglaucomatoso previo. Materiales y métodos Estudio transversal, observacional con observador enmascarado. Se incluyeron pacientes recién diagnosticados de GPAA e HTO, que no habían recibido tratamiento tópico hipotensor previo. Resultados Se incluyeron 51 pacientes con GPAA y 34 pacientes con HTO. Se encontró un valor de la PIO corneo compensada (PIOcc) significativamente mayor que el de la PIO TAG en el grupo de GPAA (p=0,0002), mientras que no se encontraron diferencias significativas entre ambos tonómetros en el grupo de HTO (p=0,1). Conclusiones La TAG parece infraestimar la PIO «real» en pacientes con GPAA sin tratamiento hipotensor previo, mientras que parece ser más exacta en pacientes con HTO (AU)

BACKGROUND AND OBJECTIVE: The corneal biomechanical properties in naïve, untreated glaucoma and ocular hypertension (OHT) eyes is interesting, because it may be a source of error in intraocular pressure (IOP) measurements by Goldmann applanation tonometer (GAT) and Ocular Response Analyzer (ORA) The main objective of this study was to evaluate the IOP values obtained using GAT and the ORA, in primary open angle glaucoma (POAG) and in OHT untreated eyes. MATERIAL AND METHODS: Observational, masked, cross sectional observational study. Newly diagnosed, untreated POAG and OHT eyes were included. RESULTS: 51 POAG and 34 OHT eyes were analyzed. Introducción: El glaucoma es la principal causa de ceguera irreversible en el mundo. La prevalencia mundial de glaucoma en personas de 40 a 80 años se estima en un 3,5 %. Objetivo: Comparar el efecto reductor de la PIO de Latanoprostene bunod (LBN) al 0,024% con Latanoprost al 0,005 % en sujetos con glaucoma de ángulo abierto (GAA) o hipertensión ocular (HTO). Metodología: Ensayo observacional de estudio de cohorte prospectivo. Resultados: Fue realizado en 28 pacientes (56 ojos) quienes fueron aleatorizados en 2 grupos paralelos (28 ojos por grupo), el grupo Latanoprost y el grupo LBN. En el grupo LBN la media de la PIO antes del tratamiento fue de 25,3 ± 6,6 mmHg y la media de la PIO luego de 1 mes de tratamiento fue de 16,5 ± 4,9 mmHg (p<0,05). En el grupo Latanoprost la media de la PIO antes del tratamiento fue de 23,6 ± 3,6 mmHg y la media de la PIO luego de 1 mes de tratamiento con Latanoprost al 0,005% fue de 15,3 ± 2,4 mmHg (p<0,05). Sin embargo, al comparar las PIOs luego de 1 mes de tratamiento con LBN 0,024% y Latanoprost 0,005% se objetiva que la diferencia en reducción de la presión intraocular entre estos dos fármacos no fue significativa (p= 0,238). Discusión: Las prostaglandinas tópicas, con su potente efecto hipotensor ocular son una importante opción de tratamiento para el glaucoma. La reducción de la PIO es la esperada con ambos medicamentos, sin embargo, no existen diferencias significativas entre ambas luego de 1 mes de uso. Con respecto a los efectos secundarios, en el grupo LBN se encontró más efectos adversos oculares.


Introduction: Glaucoma is the main cause of irreversible blindness worldwide. The global prevalence of glaucoma in people aged 40 to 80 years is estimated at 3.5%. Objective: To compare the intraocular pressure (IOP) lowering effect of 0.024% Latanoprostene bunod (LBN) with 0.005% Latanoprost in subjects with open-angle glaucoma (OAG) or ocular hypertension (OHT). Methods: Observational trial of prospective cohort study. Results: It was performed in 28 patients (56 eyes) who were randomized into 2 parallel groups (28 eyes per group), the Latanoprost group and the Latanoprostene bunod (LBN) group. In the LBN group, the mean intraocular pressure before treatment was 25.3 ± 6.6 mmHg and the mean intraocular pressure after 1 month of treatment was 16.5 ± 4.9 mmHg (p<0,05). In the Latanoprost group, the mean intraocular pressure before treatment was 23.6 ± 3.6 mmHg and the mean intraocular pressure after 1 month of treatment with 0.005% Latanoprost was 15.3 ± 2.4 mmHg (p<0,05). However, when comparing the IOPs to the 1-month treatment with Latanoprostene bunod 0.024% and Latanoprost 0.005%, it is observed, through ANOVA, that the difference in intraocular pressure reduction between these two drugs is not significant (p= 0,238). Discussion: Topical prostaglandins, with their potent ocular hypotensive effect (resulting from increased uveoscleral outflow), are an important treatment option for glaucoma. The IOP reduction is as expected with both drugs, however, there are no significant differences between the two. In the LBN group, more drug-related ocular adverse effects were found after 1 month of use.

Objetivo: Evaluar la seguridad de facoemulsificación asociada a la trabeculectomía gonioasistida modificada con tijeras. Métodos: Estudio experimental de intervención en 103 ojos (103 pacientes) con glaucoma primario de ángulo abierto. Se realizó facoemulsificación y trabeculectomía gonioasistida modificada con tijeras (51 ojos, casos) y facoemulsificación- trabeculectomía estándar (52 ojos, controles). En los casos se requirió tijera y pinza vítreo-retinal y se resecó trabéculo por abordaje interno. Se evaluó la agudeza visual, el astigmatismo inducido, la pérdida de células endoteliales y la aparición de complicaciones quirúrgicas en ambos grupos a los tres meses de seguimiento posoperatorio. Resultados: La AVSC se incrementó en ambos grupos a los tres meses postoperatorios. Sin embargo, fue mayor para la FACO-TGAM (0,81 ± 0,16vs. 0,71 ± 0,16), con diferencias significativas (p= 0,002). El astigmatismo inducido fue significativamente mayor en el grupo control (0,53 ± 1,14Dvs.1,8 ± 1,20D (p< 0.001). Las complicaciones quirúrgicas posoperatorias fueron más frecuentes en el grupo control (57 por ciento vs. 73 por ciento p= 0,14). La pérdida de células endoteliales no mostró diferencias significativas (p= 0,82). Conclusiones: La recuperación visual y el menor número de complicaciones ubican la facoemulsificación y trabeculectomía gonioasistida modificada con tijeras como técnica de elección para el tratamiento de pacientes con catarata y el glaucoma primario de ángulo abierto(AU)


Objective: To evaluate the safety of phacoemulsification associated with modified gonio-assisted trabeculectomy with scissors. Methods: Experimental intervention study in 103 eyes (103 patients) with primary open-angle glaucoma. Phacoemulsification and modified gonioassisted trabeculectomy with scissors (51 eyes, cases) and standard phacoemulsification-trabeculectomy (52 eyes, controls) were performed. In the cases, scissors and vitreoretinal forceps were required and trabeculum was resected by internal approach. Visual acuity, induced astigmatism, loss of endothelial cells and the appearance of surgical complications were evaluated in both groups at three months postoperative follow-up. Results: The uncorrected visual acuity increased at three postoperative months in both groups, however it was higher for the PHACO-TGAM (0.81 ± 0.16 vs 0.71 ± 0.16); with significant differences (p = 0.002). Induced astigmatism was significantly higher in the control group (0.53 ± 1.14 D vs. 1.8 ± 1.20 D, (p < 0.001). Postoperative surgical complications were more frequent in the control group (57 percent vs 73 pèrcent, p = 0.14), the endothelial cell loss did not show significant differences (p = 0.82). Conclusions: the visual recovery and the lower number of complications place phacoemulsification and modified gonio-assisted trabeculectomy with scissors as the technique of choice for the treatment of patients with cataract and primary open-angle glaucoma(AU)

Antecedentes y objetivo Evaluar el impacto económico en España derivado de la introducción del implante XEN®63 como alternativa quirúrgica en el manejo del glaucoma primario de ángulo abierto (GPAA) con o sin cataratas en pacientes refractarios. Materiales y métodos Se diseñó un análisis de impacto presupuestario para estimar los costes del tratamiento quirúrgico del GPAA desde la perspectiva del Sistema Nacional de Salud (SNS) español, en un horizonte temporal de1 año. Los comparadores considerados (trabeculectomía, esclerectomía profunda no perforante, válvula de Ahmed, iStent inject®, Preserflo® microshunt y XEN®45) se corresponden a los empleados en la práctica clínica pública española. Para el cálculo de la población diana, cuotas de mercado y uso de recursos en términos de visitas de seguimiento, procedimientos adicionales y complicaciones poscirugía, se emplearon datos de la literatura y se validaron con un panel de cuatro expertos. Los costes unitarios (€de2021) se obtuvieron de la base de datos ESALUD Resultados la inclusión de XEN®63 podría generar un ahorro de 2.569.737€ tras un año desde su introducción, derivado del ahorro en el coste del implante y del procedimiento (−423.120; −0,7%), de las visitas de seguimiento (−777.407€; −4,5%), de los procedimientos adicionales (−1.048.145€, −20,6%) y de las complicaciones poscirugía (−321.065€, −14,2%). Conclusiones La incorporación de XEN®63 en el arsenal quirúrgico para el tratamiento del GPAA refractario con y sin cataratas podría generar ahorros para el SNS (AU)

Objetivo La interleucina 6 (IL-6) es una citoquina proinflamatoria con efectos pleiotrópicos que se ha relacionado con el glaucoma primario de ángulo abierto (GPAA) debido a su efecto particular de protección de las células ganglionares de la retina (CGR) contra la apoptosis. Se han asociado diferentes polimorfismos de un solo nucleótido (PSN) con el GPAA. El objetivo de este estudio fue determinar si existe una asociación entre el PSN de IL-6 rs1800795 (−174 G>C) y un mayor riesgo de padecer GPAA en la población mexicana occidental. Métodos Se incluyeron 165 pacientes mestizos mexicanos no emparentados con GPAA y 108 sujetos de control. Se extrajo el ADN genómico de los leucocitos y se purificó, seguido de la genotipificación y la amplificación por reacción en cadena de la polimerasa (PCR) con sondas TaqMan® Biosystem®. Se evaluó la diversidad alélica y genotípica entre los casos y los sujetos de control. Resultados No hubo asociación estadísticamente significativa entre las frecuencias alélicas y genotípicas, ni con modelos de asociación genética dominante ni recesiva (p>0,05). Conclusiones Aunque existe un papel de la IL6 en la fisiopatología del GPAA, nuestros resultados descartan la asociación entre la IL-6 y el PSN rs1800795 mostrando no ser un índice de mayor riesgo de GPAA en la población mexicana (AU)

Conclusion Even though there is a role of IL6 in the pathophysiology of POAG, our results ruled out the association between IL-6 and the rs1800795 SNP showing not to be an index of higher risk for POAG in Mexican population (AU)

OBJECTIVE: To measure the magnitude and duration of the hypotensive effect of two prostaglandin analogues in glaucoma patients using the water drinking test (WDT). METHODS: Patients received latanoprost or travoprost every 24 h and then every 48 h. Untreated WDT were performed at 7 am and with treatment 12, 36 and 44 h after the last dose; intraocular pressure (IOP) peak, fluctuation and the difference between peak and isolated IOP measurements at consultation times were calculated. RESULTS: Forty-one eyes of 21 patients with primary open-angle glaucoma were included; 22 eyes received latanoprost, and 19 received travoprost. Mean untreated isolated IOP was 17.20 standard deviation (S.D.) 3.73 and 16.95 S.D. 2.61 mmHg and peak pressure 22.45 S.D. 2.91 and 21.58 S.D. 3.79 mmHg, for the latanoprost and travoprost groups, respectively. With treatment, peak pressure was reduced by 22.64% and 20.29% at 12 h, 18.44% and 14.64% at 36 h and 16.17% and 14.46% at 44 h, respectively. The fluctuation without treatment was 4.36 and 5.11 mmHg, and with treatment at 12 h was reduced to 2.77 and 2.89 mmHg, increasing again at 36 and 44 h. CONCLUSIONS: A hypotensive effect was evident up to 44 h after the last dose of latanoprost and travoprost, similar for the two drugs and decreasing over time. IOP fluctuation was only reduced at 12 h.

Objetivo Medir con la prueba de sobrecarga hídrica (PSH) la magnitud y duración del efecto hipotensor de dos análogos de prostaglandinas en pacientes con glaucoma. Métodos Los pacientes recibieron latanoprost o travoprost cada 24 horas y luego cada 48 horas. Se practicaron PSH sin tratamiento a las 7 am y con tratamiento 12, 36 y 44 horas después de la última dosis; se calcularon el pico de presión intraocular (PIO), la fluctuación y la diferencia entre el pico y las medidas aisladas de PIO en horas de consulta. Resultados Se incluyeron 41 ojos de 21 pacientes con glaucoma primario de ángulo abierto, 22 ojos recibieron latanoprost y 19, travoprost. La PIO aislada promedio sin tratamiento fue 17,20 desviación estándar (D.S.) 3,73 y 16,95 D.S. 2,61mmHg y el pico de presión 22,45 D.S. 2,91 y 21,58 D.S. 3,79mmHg, para los grupos de latanoprost y travoprost, respectivamente. Con tratamiento, la presión pico se redujo en 22,64% y 20,29% a las 12 horas, 18,44% y 14,64% a las 36 horas y 16,17% y 14,46% a las 44 horas, respectivamente. La fluctuación sin tratamiento fue 4,36 y 5,11mmHg, y con tratamiento a las 12 horas se redujo a 2,77 y 2,89mmHg, aumentando nuevamente a 36 y 44 horas. Conclusiones Se evidenció un efecto hipotensor hasta 44 horas después de la última dosis de latanoprost y travoprost, similar para los dos medicamentos y decreciente en el tiempo. La fluctuación de la PIO sólo se redujo a las 12 horas (AU)

The fluctuation without treatment was 4.36 and 5.11mmHg, and with treatment at 12hours was reduced to 2.77 and 2.89mmHg, increasing again at 36 and 44hours. Conclusions A hypotensive effect was evident up to 44hours after the last dose of latanoprost and travoprost, similar for the two drugs and decreasing over time. IOP fluctuation was only reduced at 12hours (AU)

RESUMEN Introducción: Las estrategias de SITA Estándar fueron aprovechadas por más de dos décadas, pero buscando aun mejorar el tiempo sin perder la calidad, se volvió a crear una nueva estrategia más corta, a la cual llamaron estrategia FASTER. Objetivo: Determinar la correlación entre las estrategias SITA Estándar 24 -2 y SITA Faster 24 -2 para evaluar el daño campimétrico en pacientes con Glaucoma Primario de Angulo Abierto. Metodología: Estudio observacional analítico. RESULTADOS: Se analizó 74 resultados de análisis del campo visual utilizando dos técnicas SITA Estándar 24 - 2 y SITA Faster 24 - 2. El tiempo de prueba para la estrategia SITA Estándar fue de 379,64 ± 72,75 segundos y para la estrategia SITA Faster 170,93 ± 49,17 (p<0,0001), probando una reducción de tiempo del 55% para la segunda. Los valores de desviación media, desviación patrón estándar y el índice de campo visual no mostraron diferencias estadísticamente significativas entre ambas pruebas. La Prueba de Hemicampo de glaucoma con el cálculo del índice Kappa de Cohen arroja un valor de 0,316 con un intervalo de confianza del 95 % de (0,098 - 0,534) lo que indica que es estadísticamente significativo. Discusión: Los resultados generales indican que es posible reemplazar la estrategia SITA Estándar 24 -2 con la estrategia SITA Faster 24 - 2. Este cambio será especialmente útil para aumentar la frecuencia de exámenes campimétricos, determinante para una detección más temprana de la progresión del glaucoma y así mejorar las conductas terapéuticas que se toman con los pacientes que acuden a nuestro servicio.


ABSTRACT Introduction: SITA Standard strategies were used for more than two decades, but still seeking to improve time without losing quality, a new shorter strategy was created again, which they called the FASTER strategy. Objective: To determine the correlation between the SITA Standard 24-2 and SITA Faster 24-2 strategies to evaluate the visual field damage in patients with Primary Open Angle Glaucoma. Methodology: Analytical observational study. Results: 74 visual field analysis results were analyzed using two techniques SITA Standard 24 - 2 and SITA Faster 24 - 2. The test time for the SITA Standard strategy was 379.64 ± 72.75 seconds and for the SITA strategy Faster 170.93 ± 49.17 (p <0.0001), proving a 55% time reduction for the second. The mean deviation, standard deviation and visual field index values do not show statistically significant differences between both tests. The Glaucoma Hemifield Test with the calculation of the Cohen's Kappa index gives a value of 0.316 with a 95% confidence interval of (0.098 - 0.534) which indicates that it is statistically significant. Discussion: The overall results indicate that it is possible to replace the SITA Standard 24-2 strategy with the SITA Faster 24-2 strategy. This change will be especially useful to increase the frequency of campimetric examinations. A greater frequency of examinations is important for earlier detection of glaucoma progression and thus improve the therapeutic behaviors that are taken with the patients who come to our service.

Objetivo: Determinar los hallazgos por eco-Doppler orbitario en pacientes con glaucoma primario de ángulo abierto según factores de riesgo aterosclerótico. Métodos: Se realizó un estudio observacional descriptivo y transversal en 300 órbitas de 150 pacientes con diagnóstico de glaucoma primario de ángulo abierto. A todos los casos se les realizó tonometría de contorno dinámico y tomografía de coherencia óptica. Se identificaron mediante interrogatorio y por el laboratorio clínico los factores de riesgo aterosclerótico: hipertensión arterial, tabaquismo, diabetes mellitus tipo 2, dislipidemia, obesidad y consumo excesivo de alcohol. Se les realizó ultrasonido orbitario y Doppler carotídeo, y solo en caso de resultar normales se procedió a evaluar mediante eco-Doppler las arterias oftálmica, central de la retina y ciliares posteriores temporales. Resultados: La edad media de los sujetos estudiados fue de 62,3 años. El 55,3 por ciento correspondió al sexo femenino y el 47,3 por ciento al color blanco de la piel. El número de factores de riesgo mostró una correlación lineal moderada, positiva y significativa con el índice de resistencia, mientras que con las velocidades dicha correlación resultó ser negativa. Todos los factores de riesgo expresaron efectos dañinos sobre la hemodinámica del flujo ocular, la presión intraocular y el grosor de las capas de fibras neurorretinianas temporales. Tras ajustar para la edad, esta negativa influencia continuó siendo relevante en la mayoría de los casos. Conclusiones: Los aspectos vasculares del glaucoma deben integrarse a la práctica clínica de esta afección, lo que ayudará a que el enfoque sea más completo, y redundará en un mejor pronóstico de la enfermedad(AU)


Objective: Determine the orbital echo-Doppler findings in patients with primary open angle glaucoma according to atherosclerotic risk factors. Methods: A cross-sectional observational descriptive study was conducted of 300 orbits of 150 patients diagnosed with primary open angle glaucoma. All the cases underwent dynamic contour tonometry and optical coherence tomography. Interrogation and clinical laboratory testing led to identification of the following atherosclerotic risk factors: arterial hypertension, smoking, diabetes mellitus type 2, dyslipidemia, obesity and excessive alcohol consumption. Orbital and carotid Doppler ultrasounds were performed, and only if they were normal they would be followed by echo-Doppler evaluation of the ophthalmic, central retinal and posterior temporal ciliary arteries. Results: Mean age of the study subjects was 62.3 years. 55.3 percent were female and 47.3 percent had white skin. The number of risk factors showed a moderate, positive and significant linear correlation with the resistive index, and a negative correlation with the velocities. All the risk factors expressed harmful effects on ocular flow hemodynamics, intraocular pressure and the thickness of temporal neuroretinal fibers. After adjusting for age, this negative influence continued to be relevant in most cases. Conclusions: The vascular aspects of glaucoma should be incorporated into the clinical management of this condition. This will make the approach more thorough and help achieve a better diagnosis(AU)

OBJECTIVE: To determine the change in aqueous humor interleukin-6 (IL-6) levels among primary open-angle glaucoma (GPAA) patients. METHODS: Studies that investigated IL-6 level in the aqueous humor in GPAA patients using the detección methods of enzyme-linked immunosorbent assay (ELISA) y multiplex bead immunoassay were retrieved. A meta-analysis was performed a determine the overall change in IL-6 of GPAA patients compared a controls. RESULTS: A total of 14 studies were selected. Analysis combining the results of studies using both detección approaches did not show any changes in the level of IL-6 in GPAA (SMD=-0,07, 95%IC -0,73 - 0,59, P=0,83), possibly due a the heterogeneity of ELISA data. Since multiplex bead immunoassay is more sensitive than convenciónal ELISA in detecting antibodies, further isolated analysis of multiplex bead immunoassay results revealed that GPAA patients had a lower level of IL-6 in the aqueous humor compared a controls (SMD=-0,40, 95%IC -0,70 - 0,09, P=0,01). Moreover, a sensitivity test also confirmed that no alteracións of results were observed in all pooled studies y pooled studies from multiplex bead immunoassay, suggesting the stabilities of our synthetic results. CONCLUSION: Because of its robustness, the pooled data from multiplex bead immunoassay was used a draw a conclusion in this study, showing that the reducción of IL-6 levels in aqueous humor was observed in patients with GPAA. Further studies are still warranted a confirm our findings.

OBJECTIVE: To design and validate a questionnaire on the knowledge of primary open-angle glaucoma (POAG) in Spanish-speaking patients. METHODS: An observational study was conducted on adults with diagnosis of POAG and who received outpatient medical care in a national reference health care institute of Peru. The questionnaire was based on a literature review and expert judgment. A pilot study was carried out on 60 patients. The items were classified and weighted according to the expert judgment. The final questionnaire was completed by 314 patients and its overall score was scaled by tertiles. RESULTS: A total of 72 items were found by primary searching. The initial questionnaire was made up of 20 items, which was tested in a pilot study. A total of 14 items were selected that were then classified into 3weighted domains: risk factors (10%), clinical aspects (36%), and treatment (54%). The questionnaire has a score range from 0 to 100 points (lowest to highest level). In patients, the mean score was 62.6±14.1. The overall score was classified into 3categories of knowledge level: low (0 to 55 points), medium (56 to 69 points), and high (70 to 100 points). CONCLUSIONS: The questionnaire provides evidence of validity to assess knowledge of POAG in Peruvian Spanish-speaking patients.

RESUMEN Objetivo: Evaluar la modificación de la capa de fibras nerviosas después de la cirugía de catarata en pacientes con glaucoma primario de ángulo abierto. Métodos: Se realizó un estudio prexperimental en dos grupos de pacientes, 31 de ellos con glaucoma primario de ángulo abierto (49 ojos) y 29 pacientes sin glaucoma (50 ojos). Se analizó el comportamiento del espesor de la capa de fibras nerviosas retinianas antes y después de la cirugía de catarata (facoemulsificación). Fue evaluado el comportamiento pre y posoperatorio de la agudeza visual, así como la presión intraocular. Resultados: La edad promedio fue 71,8 ± 8,0 años en el grupo sin glaucoma y 67,8 ± 8,9 años en el grupo con glaucoma primario de ángulo abierto. La mediana de la agudeza visual sin corrección pasó de 0,20 a 0,90 en el grupo sin glaucoma y 0,30 a 0,80 en el grupo con glaucoma primario de ángulo abierto, donde resultó la p< 0,05 en ambos casos. La mediana de la presión intraocular cayó 1 mmHg en ambos grupos. En los cuadrantes superior e inferior hubo un incremento del espesor de la capa de fibras después de la operación en ambos grupos, pero solo se observó en cuadrantes nasal y temporal en el grupo con glaucoma. Conclusiones: Después de la cirugía de catarata se produce un incremento del espesor de la capa de fibras. La opacidad impide la transmisión de la señal a la retina(AU)


ABSTRACT Objective: Evaluate the changes undergone by the nerve fiber layer after cataract surgery in patients with primary open angle glaucoma. Methods: A pre-experimental study was conducted of two groups of patients, 31 with primary open angle glaucoma (49 eyes) and 29 without glaucoma (50 eyes). The behavior of retinal nerve fiber layer thickness was analyzed before and after cataract surgery (phacoemulsification). Visual acuity and intraocular pressure were evaluated during the pre- and postoperative periods. Results: Mean age was 71.8 ± 8.0 years in the group without glaucoma and 67.8 ± 8.9 years in the group with primary open angle glaucoma. Median visual acuity without correction rose from 0.20 to 0.90 in the group without glaucoma, and from 0.30 to 0.80 in the group with primary open angle glaucoma, with p < 0.05 in both. Median intraocular pressure fell 1 mmHg in both groups. An increase in fiber layer thickness was observed after surgery in the upper and lower quadrants of both groups, but only in the nasal and temporal quadrants of the group with glaucoma. Conclusions: Fiber layer thickness increased after cataract surgery. Opacity hindered signal transmission to the retina(AU)

Objective: To determine the association of systemic diseases, as well as smoking and alcohol with primary open-angle glaucoma (POAG) in adults over 40 years age. Method: A revision of the records of patients older than 40 years aged who had been diagnosed with glaucoma, which were selected at the outpatient care dept. Information was obtained through the automated hospital information system, which included socio-demographic and clinical variables. Data base was created and was processed by SPSS V20 program. Results: 1,020 patient, 548 (53.7%) with a diagnosis of POAG, mean age of patients 73.2 ± 11.16 years, of which 193 (35.2%) were male and 355 (64.8%) female. A significant relationship was found between POAG and increasing age (p = 0.000), diabetes mellitus (p = 0.056) and hypertension (p = 0.098). While no relationship was found between POAG and cancer, smoking and alcohol intake was found. Conclusions: These results display the need of carrying out more specific studies of causal type to establish best possible partnerships and thus carry out prevention programs for early diagnosis.


Objetivo: Determinar la asociación de las enfermedades sistémicas, así como la ingesta de alcohol y tabaquismo, con glaucoma primario de ángulo abierto (GPAA) en adultos mayores de 40 años. Método: Se revisaron los expedientes de pacientes mayores de 40 años de primera vez con diagnóstico de glaucoma que asistieron al servicio de oftalmología del Instituto Nacional de Rehabilitación. La información se consiguió mediante el sistema automatizado de información hospitalaria, que incluyó variables sociodemográficas y clínicas. Se creó una base de datos y se realizó el análisis con el programa SPSS V20. Resultados: 1,020 sujetos, 548 (53.7%) con diagnóstico de GPAA, edad promedio 73.2 ± 11.16 años, y de ellos 193 (35.2%) varones y 355 (64.8%) mujeres. Se pudo establecer asociaciones significativas entre GPAA y edad (p = 0.000), diabetes mellitus (p = 0.056) e hipertensión arterial (p = 0.098), pero no se encontró asociación entre GPAA y cáncer, tabaquismo ni ingesta de alcohol. Conclusiones: Es necesario realizar estudios de tipo causal más específicos para establecer de mejor forma las posibles asociaciones, y de esta manera llevar a cabo programas de prevención para su diagnóstico a edad temprana.

El presente trabajo tiene como propósito describir la posible influencia de los factores de riesgo aterosclerótico (tabaquismo, hipertensión arterial y diabetes mellitus) en las enfermedades oftalmológicas: glaucoma primario de ángulo abierto, catarata, retinopatía diabética e hipertensiva, y degeneración macular asociada a la edad. Se realizó una búsqueda sistematizada en las bases de datos Scielo, Sciencedirect y Elsevier. El tabaquismo aumenta el riesgo de padecer, y acelera la progresión, tanto del glaucoma primario de ángulo abierto como de la catarata, la retinopatía diabética e hipertensiva y la degeneración macular asociada a la edad. La hipertensión arterial y la diabetes mellitus, además de conducir a las retinopatías correspondientes, también incrementan la probabilidad de desarrollar glaucoma primario de ángulo abierto, y son considerados factores de mal pronóstico al favorecer su progresión. El control del tabaquismo, de la hipertensión y de la diabetes mellitus mejorará el pronóstico de estas afecciones oftalmológicas, y en algunos casos evitará o retardará su aparición(AU)


The purpose of the present work is to describe the possible influence of the atherosclerotic risk factors (smoking, hypertension and diabetes mellitus) in the ophthalmological illnesses: primary glaucoma of open angle, waterfall, diabetic and hypertensive retinopathy, and macular degeneration associated to the age. Eye diseases such as glaucoma, cataract, diabetic and hypertensive retinopathy and age-related macular degeneration are closely related to atherosclerotic risk factors: smoking, hypertension and diabetes mellitus. Knowing how these factors influence ocular structures will facilitate the diagnosis and treatment of these conditions. Systematic search was made in Scielo, Sciencedirect and Elsevier databases. Smoking increases the risk of suffering and accelerates the progression of primary open-angle glaucoma, as well as cataract, diabetic and hypertensive retinopathy, and age-related macular degeneration. Hypertension and diabetes mellitus, in addition to leading to corresponding retinopathies, also increase the likelihood of developing open-angle primary glaucoma; they are even considered poor prognosis factors since they prompt its progression. The control of smoking, hypertension and diabetes mellitus will therefore improve the prognosis of these ophthalmological conditions, and in some cases will prevent or delay their onset(AU)

Se realizó una revisión bibliográfica con el objetivo de analizar la producción científica publicada por autores cubanos sobre glaucoma primario de ángulo abierto en los últimos 35 años, para lo cual se consultaron artículos científicos de las revistas médicas que aparecen en las bases de datos Medline (PubMed), SciELO Regional, CUMED, IBECS y LILACS. En total se recuperaron 33 artículos, de los cuales 27 fueron originales, 4 revisiones bibliográficas y 1 presentación de caso. Los resultados reportados en dichos estudios evidencian los logros alcanzados por los investigadores cubanos sobre esta importante afección oftalmológica, especialmente en lo que concierne a los tratamientos antiglaucomatosos y al pesquisaje temprano de pacientes con factores de riesgo glaucomatoso. Sin embargo, no se encontró ninguno realizado con el propósito de relacionar esta enfermedad con los factores de riesgo aterosclerótico, lo que podría servir de base para la realización de nuevas investigaciones(AU)


A literature review was made to analyze the scientific production published by Cuban authors on the primary open angle glaucoma in the last 35 years. To this end, the scientific articles from the medical journals included in Medline (PubMed), Regional SciELO, CUMED, IBECS and Lilacs. Thirtty three articles were recovered in total, 27 were original manuscripts, 4 literature reviews and one case report. The reported results of such studies show the achievements attained by the Cuban researchers in the field of this important eye disease, particularly the anti-glaucoma treatments and the early screening of patients with glaucomatous risk factors. However, no paper about the relationship of this disease with atherosclerotic risk factors was found, which would have serve as a basis for the conduction of new research studies(AU)

Sin embargo, no se encontró ninguno realizado con el propósito de relacionar esta enfermedad con los factores de riesgo aterosclerótico, lo que podría servir de base para la realización de nuevas investigaciones(AU)


A literature review was made to analyze the scientific production published by Cuban authors on the primary open angle glaucoma in the last 35 years. To this end, the scientific articles from the medical journals included in Medline (PubMed), Regional SciELO, CUMED, IBECS and Lilacs. Thirtty three articles were recovered in total, 27 were original manuscripts, 4 literature reviews and one case report. The reported results of such studies show the achievements attained by the Cuban researchers in the field of this important eye disease, particularly the anti-glaucoma treatments and the early screening of patients with glaucomatous risk factors. However, no paper about the relationship of this disease with atherosclerotic risk factors was found, which would have serve as a basis for the conduction of new research studies(AU)
